Could Bilberry Help Protect the Aging Brain?

What microcirculation, anthocyanins, and vascular dementia can teach us about keeping the brain strong


If I told you there was an herb traditionally associated with eyesight, you might immediately think about the eyes.


But what if the more interesting story is actually about the blood vessels behind the eyes…and throughout the brain?


That is the question that has changed the way I think about Bilberry (Vaccinium myrtillus).


Bilberry has long been associated with eye health. But when we look more closely at its traditional applications and modern research, another theme begins to emerge:

microcirculation.


And that matters enormously when we're talking about protecting the aging brain.


Your brain depends on its smallest blood vessels


We tend to think about cardiovascular health in terms of the large vessels: blocked arteries, heart attacks, strokes and cholesterol.


But your vascular system doesn't end with the arteries.

Large arteries become smaller arteries.

Smaller arteries become arterioles.


Arterioles become an extraordinary network of microscopic capillaries responsible for delivering oxygen and nutrients to tissues throughout the body.


The brain is particularly dependent on this system.


Cerebral small-vessel disease involves damage to the brain's small blood vessels and is a major contributor to vascular cognitive impairment and vascular dementia. Researchers increasingly recognize dysfunction of the vascular endothelium, the cellular lining of these vessels, as an important part of this process.


That gives us an important perspective on brain health:

Protecting the brain also means protecting the circulation that feeds the brain.

What is vascular dementia?


Vascular dementia is cognitive impairment caused by conditions that interfere with blood flow to the brain.


Sometimes that damage occurs dramatically, such as following a stroke.

But vascular injury can also accumulate more quietly through disease affecting the brain's small vessels.


Over time, compromised vascular function can contribute to changes in memory, processing speed, executive function and other aspects of cognition.


This is one reason cardiovascular risk factors such as hypertension, diabetes and obesity have also been associated with cognitive decline, changes in brain structure and dementia risk.


So when someone asks me:

“What can I do now to help keep my brain strong as I age?”


I think circulation deserves a seat at the table.

And that's where Bilberry becomes interesting.


Bilberry is not merely an “eye herb”


Bilberry contains a remarkable family of plant compounds called anthocyanins.


These are the pigments responsible for many of the deep red, blue, purple and almost-black colors we see in foods such as blueberries, blackberries, black currants, cherries, purple grapes and purple cabbage.


Bilberry is particularly rich in them.


But anthocyanins aren't interesting simply because they're “antioxidants.”


Research suggests their biological effects may involve endothelial function, nitric-oxide signaling, oxidative stress, inflammation and vascular regulation.


A 2024 systematic review of 20 intervention studies examining both cognition and vascular function found beneficial effects in several cognitive measures, with some imaging studies also demonstrating increased blood flow in brain regions associated with those cognitive functions. The authors were appropriately cautious, however: effects on blood pressure and endothelial markers were inconsistent.

That distinction matters.


We're not talking about a magic berry that “prevents dementia.”

We're asking a much better question:


Could regularly supporting vascular health be one of the variables that helps us build a healthier environment for the aging brain?


I think that is worth exploring.


The anthocyanin–brain connection gets even more interesting


A large 2021 systematic review examined 49 human intervention studies involving berry anthocyanins.

The researchers found evidence of improvements in some measures of memory and reported beneficial effects on vascular measures including endothelial function. However, they also concluded that whether the vascular improvements actually explain the cognitive benefits remains uncertain.


Then, in 2024, another systematic review specifically examined studies that measured both vascular and cognitive function.


Fifteen of the 20 included studies found a positive effect on at least one cognitive test. Some functional MRI studies showed increased blood flow in brain regions associated with verbal and working memory.

But here's where good science requires some humility.


A 2025 systematic review and meta-analysis found encouraging signals across the anthocyanin literature, but when eligible studies were pooled statistically, improvements in several specific memory domains were not significant. The researchers emphasized substantial differences between studies.


That's precisely why I would never say:

“Take Bilberry so you don't get dementia.”


We simply don't have evidence for that.


Instead, I would say:


“Let's look at the biology we're trying to protect.”


The eye may give us a window into the bigger story


There's another reason I find Bilberry fascinating in a brain-health conversation.

The retina is neural tissue with an extraordinarily rich microvascular network.


So perhaps it shouldn't surprise us that many of Bilberry's best-known applications involve the eye.

But once we stop thinking about Bilberry as an “eye supplement” and start thinking about it through the lens of microcirculation, suddenly its seemingly unrelated historical applications begin to make more sense:


Eye health → capillary integrity → venous circulation → edema → vascular function → healthy aging.


There is a common thread running through much of this story.


Blood flow.


What about the lymphatic system?

This was one of my favorite discoveries while preparing my recent Bilberry lecture.

The vascular and lymphatic systems don't operate independently.


Fluid continually moves out of capillaries into the interstitial space surrounding our cells, and the lymphatic system helps manage that fluid and return it to circulation.

If excessive fluid escapes from the vascular compartment, the lymphatic system has more work to do.

So another way of thinking about vascular support is:


Healthier capillaries

Less excessive leakage

Less interstitial fluid accumulation

Less edema

Less burden on lymphatic clearance


That doesn't mean Bilberry has been proven to “detox the lymph” or directly stimulate lymphatic flow.

It's a more sophisticated idea:

Sometimes supporting lymphatic function may involve reducing the amount of excess fluid the lymphatic system has to manage in the first place.

That's why I increasingly think of Bilberry not simply as an eye herb, but as a potential vascular-lymphatic support herb.


So, should I take Bilberry every day for my brain?


Here's my answer:


Possibly...but not because Bilberry has been proven to prevent vascular dementia.


I would consider Bilberry as one potential piece of a much larger brain-health strategy, particularly when I see other clues suggesting that microcirculation deserves attention.

And this is where the questionnaire we created becomes useful.


What is your microcirculation telling you?


Ask yourself:

  • How many hours each day do you spend looking at screens?

  • Do your eyes feel tired, dry, strained or blurry by evening?

  • Has your ability to adjust from light to darkness changed?

  • Do you bruise easily or notice tiny broken capillaries?

  • Do your legs become heavy, achy or swollen later in the day?

  • Do your socks leave deep marks around your ankles?

  • Do you have spider veins or varicose veins?

  • Do you frequently feel puffy or retain fluid?

  • Does swelling improve overnight or when your legs are elevated?


    And perhaps most importantly:

  • Are you actively thinking about preserving your vision, circulation and cognitive function as you age?


None of these questions diagnoses poor cerebral circulation, vascular dementia, or a need for Bilberry.

They're clues.

They encourage us to look at the body as an interconnected vascular system rather than waiting for one organ to develop a problem.



Food first: eat more purple


There is another part of this story that I don't want us to miss.

You don't need a supplement bottle to start increasing your intake of anthocyanins.


Eat the colors.


Bilberries. Blueberries. Blackberries. Black currants. Cherries. Purple grapes. Purple cabbage. Purple sweet potatoes. Aronia berries.


That's one of the simplest brain-health habits I can recommend:


Eat purple. Often.


Anthocyanins are widely distributed throughout the food supply, and the research on cognition and vascular function includes numerous anthocyanin-rich foods, not simply Bilberry extracts.


A standardized Bilberry extract becomes a different conversation when we're looking for a known and reproducible anthocyanin intake.


That's why standardization matters.


Brain health is vascular health, too


When people think about preventing cognitive decline, they often think about neurons.

I want us to think about something else, too:


What feeds those neurons?


Blood vessels.


Thousands upon thousands of microscopic vessels continuously delivering oxygen, glucose and nutrients to extraordinarily metabolically active tissue.


That leads me to a much broader view of Bilberry.


Bilberry is not merely an eye herb.


It is an herb with interesting applications to:


Microcirculation.

Capillary integrity.

Connective tissue.

Vascular aging.

Vascular-lymphatic physiology.


And because the brain depends so heavily on healthy circulation, I think Bilberry deserves a place in the larger conversation about how we protect the brain across a lifetime.


Not as a cure.

Not as a guarantee.

Not instead of managing blood pressure, blood sugar, exercise, sleep, smoking, diet, cardiovascular risk or appropriate medical care.


But perhaps as one more useful variable in a much bigger strategy.


And if your goal is to do everything reasonably within your power to keep your brain strong for decades to come, I think that's a conversation worth having.


Start with this question:

What is my microcirculation telling me today, and what can I do to protect it before it becomes a problem?

That may turn out to be a much more important brain-health question than we realized.
